Good Faith Estimate Notice

Your Right to a Good Faith Estimate
Under the No Surprises Act, you have the right to receive a Good Faith Estimate explaining how much your medical or mental health care will cost. If you do not have insurance or choose not to use your insurance, you may request a written estimate of the expected cost of services before scheduling or beginning treatment. A Good Faith Estimate will include the expected charges for services provided by your therapist as well as any other reasonably anticipated costs related to your care.

When You Can Request an Estimate
You have the right to receive a Good Faith Estimate:

  • When you schedule services at least 3 business days in advance
  • When you request an estimate before scheduling services
  • When ongoing services are expected as part of treatment

For ongoing psychotherapy services, the estimate may include the anticipated number of sessions and estimated total cost of care over a period of time.

If Your Bill Is Higher Than the Estimate
If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill. You may initiate a dispute resolution process with the U.S. Department of Health and Human Services (HHS). You must start the dispute process within 120 calendar days of receiving the bill.
